Issued with inner sleeve, printed one side only, with four boxed panels of information. At the top is 'IMPORTANT!' At the bottom, 'PLAYING SPEED 33 1/3 r.p.m' British patent No. 800513. In the lower left is a date-code in the form MMY, so that the first sleeves have a code "125" for 12=December [196]5.
